2011 Open Enrollment for Life and AD&D Insurance |
| Open Enrollment available until February 28, 2011 | | NYSCOPBA offers our Members Term Life and Accidental Death & Dismemberment (AD&D) insurance through payroll deduction. Members may apply for Term Life and AD&D insurance at any time, however during the open enrollment period members who have never applied for coverage may enroll without any medical questions. Members who currently are enrolled may increase their coverage by 1 level, again with no medical questions. |
| | Members who currently participate: | | You will be mailed the necessary information and forms to increase your life insurance 1 level and/or add spousal and child coverage. If you do not receive the forms by mail, you may call Norvest Financial Services at (888) 869-8252 and have the forms mailed to you. | | Increase Limits |  Members may increase their life insurance 1 level ($50,000) to a maximum of $300,000
Members may increase their spouse life insurance 1 level ($25,000) to a maximum of $50,000
Existing participants who do not currently have spouse and child coverage may purchase coverage subject to the following limits: Spouse $50,000, Child(ren) $4,000
Effective date of increase will be when the new premium is reflected in your pay check |
