Inmate charged out of the Room and punched Sr SHTA in the head. Additional SHTAs responded and placed inmate in 5 pt restraints.

Inmate charged out of the Room and punched Sr SHTA in the head, Additional SHTAs responded and placed inmate in 5 pt restraints, SHTA injured his hand during the restraint. The inmate was combative during the entire incident.

Officer was doing feed up when inmate reached out of his cell with his right arm and attempted to punch Officer in the face. Officer blocked inmate, causing inmate to strike him in the right hand.
