Inmate had cut on finger and flicked blood onto CO, striking CO’s shirt, pants and into eyes.

Officers breaking up a 2 man fight involving a weapon and the Inmate with the weapon punch the officer in the face.

Using a shampoo bottle, Inmate squirted toilet water on CO.

Patient punched and scratched SHTA causing shoulder swelling and pain and scratches along both forearms.

Patient assaulted SHTA in dining area, resulting in injuries to SHTA’s wrist, knee and back.

Inmate punched Officer in the face, resulting in a broken nose.
