While in the dayroom, patient charged out and kicked SHTA. During restraint patient spit in SHTA’s face in both eyes and nose.

Inmate for no apparent reason threatened staff, and as SHTA approached, the inmate punched SHTA in the face.

Patient punched SHTA while separating the patient from fighting with another patient.

Inmate was yelling at staff, SHTA attempted to talk to the inmate when the inmate punched SHTA in the left side of his face.

While being escorted, inmate turned and assaulted COs with elbows to their upper bodies.

While escorting inmate, inmate turned around and punched the Officer in the head, neck and face area 4 to 5 times.
